發病初期通常沒有明顯症狀，常因例行體檢、或曾與結核病患接觸而接受胸部Ｘ光檢查才被診斷出來，此時既沒有症狀，相當比例的病人並不會就醫；之後病情逐漸由輕加重，大約經過經過半年一年，肺內開始形成空洞，具有傳染性，也開始有了長期咳嗽、喀痰等症狀；若此時仍未能就醫、或就醫而未及時得到正確診斷、或已診斷卻未接受正確治療，則病情繼續加重，再經過半年一年演化成重度肺結核，食慾不振、消瘦、倦怠無力、發燒、重的時候可能還會有午後潮熱，夜間盜汗，甚至於胸痛、喀血等症狀一一出現，最後導致死亡。肺結核病人不接受治療，5年內約有1/2的病人死亡，1/4的病人在5年後仍持續在散播結核菌，另1/4的病人 5年中肺結核轉為不活動，結核菌再度進入冬眠，伺機而動。
